The Serbian cut Alcaraz's lead at the top of the ATP to 20 points, and will reclaim the top spot if he wins the first half at the US Open.
Djokovic has not lost in the first round of the Grand Slam since the 2006 Australian Open.
Meanwhile, Danish Holger Rune moved up to No.4, kicking Greek tennis player Stefanos Tsitsipas to drop three places.
Alexander Zverev moved up five places to 12th place after reaching the semifinals in Cincinnati.
