Smart shuffle was first introduced in 2023. As the name implies, this feature will mess up your playlist, but not like the usual shuffle feature. When this feature is active, users will hear songs that are not added to the playlist.
This happens because Spotify combines song recommendations when you play songs from a playlist. These recommendations will be added among the songs you like. Some may like this feature, while others don't.
The reason is, Spotify can recommend songs that don't match the taste of its users. Imagine if you're playing a favorite playlist, then a song you don't like appears, it will interfere with your listening experience.

Spotify adds an explanation at the bottom of its feature that reads, "If it's enabled, you can select Linear, shuffle, and Smart shuffle playback modes on your playlist. Disable to remove the Smart shuffle option."
