Abe appears to have had a heart attack, NHK and Kyodo news agency said. A gunshot was heard and a puff of white smoke was seen as Abe made a speech outside the train station. A NHK reporter at the scene said they could hear two explosions in a row during Abe's speech.
Prime Minister Fumio Kishida condemned the shooting during the high house election campaign in Nara City to be held on Sunday, as an unacceptable attack on the basis of Japanese democracy.
Earlier, a hospital official said Abe appeared in a heart attack when he was flown to hospital after initially conscious and responsive.
Separately, Japanese police said a 41-year-old man suspected of shooting had been arrested. NHK quoted the suspect, Tetsuya Yamagami, who told police he was not satisfied with Abe and wanted to kill him.
Abe served two terms as prime minister to become Japan's longest-serving prime minister before stepping down in 2020 for poor health reasons.
